Forms All forms are in PDF format and may be filled out electronically. The form fields are not highlighted. Box 5190 Kent, OH 44242-0001 Registration Forms Alternate Course Request Form This form must be submitted prior to taking a course for credit towards your degree that is not listed on the IAKM advising sheets (see the Program Planning and Advising Forms below). A course syllabus must be submitted with the form. Individual Investigation Approval Form This form must be submitted for approval to enroll in an Independent Investigation (IAKM 61096). Once you have all approvals, the IAKM Office will register you for your Independent Investigation section. All arrangements for an Independent Investigation should be made prior the start of the semester in which the investigation will take place. Internship Approval Form This form must be submitted for approval to enroll in an Internship (IAKM 60092). Once you have all approvals, the IAKM Office will register you for your Internship section. All arrangements for an Internship should be made prior the start of the semester in which the internship will take place. Practicum Approval Form and Contract This form must be submitted for approval to enroll in a Practicum (IAKM 60692). Once you have all approvals, the IAKM Office will register you for your Practicum section. All arrangements for a Practicum should be made prior the start of the semester in which the practicum will take place. Workshop Credit Request This form must be submitted and the workshop approved prior to registration for the workshop. Once approved, you then register for the workshop as described on the Workshops page. Remember : you may use up to 4 hours of credit for workshops towards your degree. Workshops may be used for credit only if the content does not overlap course content. For instance, if you would like to take CS 57105 - Web Design and Programming, you may not earn credit for a workshop that covers topics from that class (HTML, JavaScript, and Perl). Master's Project Master's Project Handbook The Master's Project Handbook is a comprehensive guide to completing a master's project, including prerequisites, requirements, deadlines and sample documents. Master's Project Approval Form This form is to be submitted to obtain approval to begin your Master's Project. The form should be submitted with all appropriate signatures, abstract outlining the project and completed proposal by the deadlines outlined in the Master's Research Project Handbook . Master's Project Proposal Elements This form describes suggested components for students to include in their Master's Research Project Proposal. Thesis Thesis Handbook The Guide to Preparation of Thesis includes comprehensive information for students completing their Master's Thesis. The guide outlines thesis requirements and deadlines, as well as the order and process of completing a Master's Thesis. Thesis Elements This form describes recommended elements for students to include in their Master's Thesis Proposal. Thesis Preparation Approval Form This form is to be submitted with the final draft of your thesis for final approval of your thesis. Advising Sheets These advising forms allow you to plug in your coursework to keep track of your progress KM Advising Sheet UXD Advising Sheet Course Lists These Course Lists contain all of the courses for each concentration: List of IAKM-Related Courses Change of Advisor Form This form is to be submitted to the IAKM Office upon completion when you would like to change faculty advisors whether you are changing concentrations or for other reasons. IAKM Transfer Credit Request Form This form should be submitted if you have credit from a master's program from another university that you would like to apply to your IAKM degree.
